Since completing MRCGP in 2008 and PGDip Diabetes in 2012 in London, I have been providing high quality Diabetes care in Primary Care. Currently working as PCN Clinical Director and Brent Borough Partnership Diabetes Clinical lead has given me opportunity to contribute towards population health management through national and local programs like Northwest London Diabetes Enhanced Services, REWIND ( Diabetes Remission), National Diabetes Prevention Programme, Fresh start programme ( Virtual group consultations) and Brent Health Matters while addressing health inequalities in diverse population of Brent. During this process I have trained and set up a PCN Diabetes Support team with newer ARRS roles of clinical pharmacists, dieticians, health coaches and care coordinators.
