Transition tightrope – supporting confident step change

This session looks at the best and not so optimal of the current systems used whilst transitioning care of a young person with diabetes from paediatric to adult services.  With speakers including consultants from paediatric and adult services, views from young people who’ve transitioned and families preparing their young person for transition, this session will involve input from every stakeholder in the process.  Focusing on the vast range of needs a YPD may have at this vulnerable time in their life and how, by working together, we could improve the care and support offered which will result in positive clinical impacts too.
