We would like to share the experience of enacting and maintaining transformational change within Specialist Diabetes services along with the outcomes achieved for people with Diabetes and our Health Economies.

We would like to share our focus on the use of Information Technology, communication, education, commissioning and integration of care in multiprofessional clinical practice.
