STPs – North West London – reality of delivering transformation in diabetes care – toolkit for success

The North West London Diabetes Transformation Programme is successfully improving key metrics and outcomes for people living with diabetes and those at risk of developing type 2 diabetes across 8 CCGs and a population of over 2 million residents. In the past three years the team have achieved clinical transformation at scale, referred over 12,000 people into the national diabetes prevention programme, run a digital structured education pilot and have developed an outcomes-based diabetes service specification, as well as improving attainment of 3 treatment target metrics.

Learning outcomes:

  • How to build a business case for CCG investment: getting the balance right in terms of quality improvement and financial savings is challenging. This session will provide some useful tips and insights about the approach we have taken
  • Digital innovation: digital transformation and innovation has been a key part of our work and I’ll be explaining some of our approaches
  • Building blocks: successful transformation is like a house of cards – remove one and the house falls down. This session will explain key components needed for success
  • Stakeholder engagement: some of the lessons we’ve learned along the way about lining up your ducks correctly
  • Building an outcomes framework: our work to date in developing an integrated outcomes based specification
