Improving Diabetes Care: Masterclass for Primary Care staff in delivering the 8 Care Processes and 3 Treatment Targets
)
Did you know thousands of people living with diabetes miss out on life-saving checks every year?
The National Diabetes Audit shows that every year thousands of people living with diabetes miss out on key health checks that can prevent serious complications. These key health checks are known as the 8 Care Processes, which include the 3 Treatment Targets (HbA1c, blood pressure and cholesterol).
Most routine diabetes care is carried out in Primary Care, a workforce that is under pressure to deliver more and more high quality care. Like all parts of the NHS, it is undergoing huge changes, one of which is that it is becoming a ‘newer’ workforce as more and more experienced Practice Nurses and HCAs are leaving the profession, Primary Care is left with a knowledge and confidence gaps. Without a thorough understanding of WHY we carry out certain health checks, it is easy to see how aspects can be missed and targets not met.
It is easy to forget to see the ‘diabetes review’ as a person.
