Self-management support in people with multiple long-term conditions (multimorbidity)

Self-management and self-management support are associated with improved health outcomes across a range of health conditions, including diabetes. However, people with diabetes commonly live with multiple long-term conditions (MLTCs), which can pose additional challenges for self-management and supporting self-management.
MLTCs is defined as the co-existence of two or more long-term conditions in a person. MLTCs are a growing public health concern, with increasing numbers of people living with two or more health conditions in the UK. This talk will explore the role of self-management support for people living with MLTCs, from both an individual and healthcare perspective.
