CaReMe-O in primary care- is it really ‘out with old, in with the new’?

The evolving landscape of cardiovascular, renal, and metabolic (CaReMe) care is transforming the way we approach complex, multimorbid patients in primary care. CaReMe-O, a novel framework integrating holistic risk management and outcome-based strategies, challenges traditional disease-siloed approaches. But does this really mean replacing established practices, or is it about building on existing strengths with new tools and evidence? This session will explore how CaReMe-O can be implemented in day-to-day general practice, its potential to streamline care pathways, and the practical balance between innovation and continuity.
