A sustainable approach to delivering the Healthier You NHS Diabetes Prevention Programme in Mid and South Essex

M&SE’s progress towards sustainability of the NHS DPP has been an evolving process shaped by feedback from our regions’ health care professionals. This presentation highlights the key steps the ICS implemented during this process to support its’ aim of embedding the NHS DPP into primary care practice.

It will also look at the comms being used to help maintain referral momentum and to supporting equality of access in preparation for the Framework 3 contract next year.
