·        How Hampshire and Isle of Wight NHS DPP team engaged key stakeholders to support NHS DPP rollout

  • Our approach to ensure quality referrals were maintained during the COVID-19 pandemic
  • Deep dive review of individual GP practices and planned projects to support the lower-performing practices
  • How we set up and are delivering 2 projects to support NHS DPP referrals:

o   Health Care Assistant project have increased referrals and engaged lower referring practices


o   Secondary Care Referral Pathway Project is testing an alternative referral route to primary care and to make every contact count.  

  • Identify how HIOW have engaged key stakeholders and primary care
  • Identify key strategies used to maintain referrals during times of increasing pressure on primary care
  • Understand how the Healthcare Assistant Pilot has supported engagement and increased referrals from lower referring practices and lessons learned.
  • Learn how the secondary care referral pathway pilot was developed, being delivered and lessons learnt.
