In this session, explore the process and content of moving from 6 locally commissioned diabetes services to one in Sussex. We will share how this has been achieved, the challenges, pitfalls and recommendations of how colleagues might undertake this themselves. We intend to discuss our approach and how this links with ICS and NHS England values, including reducing health inequalities.
